Get the new hardware, plan the steps for install/conversion, create test cases, run a test conversion, adjust conversion procedures, run your test cases and, once all issues are resolved and documented, schedule downtime, blow away the target system so you have a clean platform again and pull the trigger. You run in to the same problems (maybe more because you have time to find them) but you aren't under the pressure of a downtime window to fix them. You can also get your users to do "acceptance" on the system before the real conversion (spread the responsibility). That makes the actual conversion much less stressful and more successful.
