Can you please run the installer again in debug mode (ubiquity -d from a
terminal window) and attach /var/log/syslog, /var/log/partman, and
/var/log/installer/debug following the instructions in
http://wiki.ubuntu.com/DebuggingUbiquity/AttachingLogs
