I'm experiencing similar simptoms. I filed a bug report here (https://bugs.launchpad.net/ubuntu/+source/pulseaudio/+bug/296513)
I attached, there, a lot of stuff, uname, asound/cards, var/log/messages and I don't remember what else. I don't know wether it's a duplicate because I'm not getting any dialog window claiming any error from the System-> preferences -> audio panel.
