I suggest you rename the /sbin/hdparm to /sbin/hdparm-test so you can be sure nothing else is calling it. Then manually set hdparm-test -B 128 /dev/sda and see if it still spins down. If so, it must be the kernel that is not handling your hard drive, or the hard drive is handling spin down on its own. Please give all hardware details, hd controller (lspci output) and hd model (dmesg output).
