Confirmed on a fully updated Kubuntu Hardy (recently upgraded from Gutsy) with Firefox 3 running the Plastik KDE theme.
Installing the gtk2-engines package provided a number of Gtk+ themes. Then, going into "System Settings", under "Look & Feel" choosing "Appearance", then entering the "GTK Styles and Fonts" panel and choosing Clearlooks in the "GTK Styles" section did provide properly functioning form controls once I restarted Firefox. I decided to install gtk2-engines-qtcurve and choose QtCurve both as my KDE style and as the GTK+ style described above. However, the GTK+ style must be chosen explicitly: the "Use my KDE style in GTK applications" does not manage to properly enable the QtCurve styling for Firefox. This is itself probably some kind of bug or omission.
