Same problem here with Karmic in a lab with 17 preseeded P4/Intel Chipset PCs. 
Some of them are booting the first time, some of them on second attempt, 
totally irregulary, but with the symptoms described above. In the second lab 
with DualCore machines everything is fine under the exact same software 
configuration.  I removed modemmanager und apparmor because the last screen 
messages seemed to be related to them but no success.
I have no clue or time where to dig deeper so I downgraded the lab back to 
jaunty -> problem gone...
