I thought I had this licked... but now it's returned. Worse, the workaround above (export PULSE_NO_SIMD=1 and remove the ~/.pulse folder) now only reduces the problem, which now seems to worsen over time.
In addition, things that previously caused the problem to abate, like setting the levels in alsamixer, no longer work.
