etc/modprobe.d/blacklist-kernel-nouveau.conf: this file is not part of a
stock Ubuntu system.  Do you have this file on your root filesystem?  If
so, 'dpkg -S /etc/modprobe.d/blacklist-kernel-nouveau.conf' should tell
you what package it belongs to.

If it's not present on the rootfs, does regenerating the initramfs again
with 'update-initramfs -u' get rid of the copy in the initramfs?

If none of this solves the problem, you'll want to grep for this
filename in /etc/initramfs-tools and /usr/share/initramfs-tools, to find
what's creating it.
