etc/modprobe.d/blacklist-kernel-nouveau.conf: this file is not part of a stock Ubuntu system. Do you have this file on your root filesystem? If so, 'dpkg -S /etc/modprobe.d/blacklist-kernel-nouveau.conf' should tell you what package it belongs to.
If it's not present on the rootfs, does regenerating the initramfs again with 'update-initramfs -u' get rid of the copy in the initramfs?
