To follow up on this - I talked to Colin Watson, who suggested that the EFI boot wasn't expected to work well on this hardware. I got installation to work by remastering the install CD - copy the contents to a temp dir, them run mkisofs to generate an .iso from the tmp dir. The resulting CD boots using the 'Windows' boot option provided by the Macbook's firmware, and avoids the "Select CD-ROM Boot Type" prompt.
