Hmm, could it be a PulseAudio problem then? There are instructions for how to create a log here: https://wiki.ubuntu.com/PulseAudio/Log but that log would only be helpful if 1) volume is loud immediately after having restarted pulseaudio 2) you can move the slider, that fixes the problem, and you tell the time that this happened.
