Thanks for reporting this bug, and helping make Ubuntu better. You
shouldn't have to add the "Include /etc/phpmyadmin/apache.conf" line to
/etc/apache2/apache2.conf because that file is symlinked to
/etc/apache2/conf.d, and apache2 is restarted as part of the phpmyadmin
package install.
Can you try the Server Guide instructions again, and confirm this is the
case for you. On a clean install of Ubuntu 10.10 that is the case for
me.
