I was able to recreate this by killing a ubiquity process when
installing from an ISO dated 20110415. I received a dialog that
ubiquity had crashed with information about how to manually report a bug
and then was returned to a desktop session. No crash report was written
to /var/crash/ which would make reporting a bug easier.
