This seems more complicated than I thought. Sometimes it is possible to launch from the command line and get a working menu. However, clicking on a menu item greys out the menu. Further instances of gvim launchpad from the command line do not display the menu at all or display the unclickable greyed out menu. Launching gvim from the dock works fine, and resets things so instances launched from the command line may have the menu displayed.
