It turns out that this is a linux kernel issue, specific to the Intel DP43BF mainboard, and perhaps some others. The problem affects the entire PCI bus (but not PCIe devices). No device plugged into a standard PCI slot is detected by the kernel unless pci=assign-busses is passed to the kernel by the boot loader. see https://bugzilla.kernel.org/show_bug.cgi?id=18412
This is at least partially an Intel problem, but the Linux kernel should be able to detect this issue and work around it, as Windows does.
