I can confirm this bug and state it relates to use of excessively long
filenames in the Ubuntu Server 12.04 standard release CD iso image. The
transfer from ISO9660 to vfat filenames therefore results in one or two
filename truncations taking place.
This careless (on the part of the Canonical Server teams' packagers)
makes it an unnecessarily obscured process to install the Ubuntu Server
release using a USB flash drive.
