Can you try attaching the files /var/log/dmesg, /var/log/daemon.log, /var/log/user.log and /var/log/kern.log to this report, straight after it has happened again? (If you are busy at the time, can you take a copy of the files, and then upload them when you get round to it?)
I'm hoping that something will be logged by the system as it fails.
