Raymond's fix did not work for me. What worked was commenting the sections in
/usr/share/pulseaudio/alsa-mixer/paths/analog-output.conf and
/usr/share/pulseaudio/alsa-mixer/paths/analog-output-headphones.conf:
#[Element Master]
#switch = mute
#volume = merge
#override-map.1 = all
#override-map.2 = all-left,all-right
