I've looked over the driver and can't find anything obviously wrong. As
far as I can see the device ought to be getting configured the exact
same way when it fails as when it works.

I made another build with some extra debug statements in the driver that
should reveal whether certain exceptional conditions are occurring. If
this doesn't reveal anything then I'm out of ideas that don't involve
time-consuming reverse engineering efforts. It may be that this
particular dongle has some quirky behavior that the proprietary driver
knows how to work around or deal with, but we are unfortunately not
privy to any such information.
