Same problem - coming back from resume, hibernate, suspend, after one good resume.. Subsequent resumes fail with end_request i/o errors.. Is it swap disk related? This did not happen a few iterations before 12.xx.. I believe it was all right in 11.xx.. But a kernel update definitely created this issue in my opinion.. Also, nothing wrong with hd, although that was at first a concern and possibility. Tested, and seems fine in Windows and with other tool examinations.
