By the way, I'm using kernel 3.15.3, so it's probably not fixed upstream. If my theory turns out to be true, then there is a kernel bug (allowing rfkill to interrupt the fw transfer), but also an easy userspace fix (delay rfkill-restore until hci0 is up).
