It appears you are using the 16.04.0 d-i release, rather than the one
from updates. That means you do not see any benefits and bugfixes to the
installer since the 16.04.0 original release. And an older kernel is
used to boot the d-i.

Was it intentional for you to use a superseded installer?

I would have expected for new installation to use the latest stable
installer for xenial series, from xenial-updates pocket:

E.g.
http://de.ports.ubuntu.com/dists/xenial-updates/main/installer-s390x/current/images/

Which currently refers to http://de.ports.ubuntu.com/dists/xenial-
updates/main/installer-s390x/20101020ubuntu451.6/images/

You can see all currently available stable installer builds at:
http://de.ports.ubuntu.com/dists/xenial-updates/main/installer-s390x/
With current directory being a symlink to the most recent one.
