When connecting or disconnecting headphones and thus switching sink, a
volume notification pops up showing the volume level of the now inactive
sink instead of the new active sink.

This also happens on automatic sink switching (via switch-on-connect) to
or from bluetooth headphones.

In the bluetooth case, if the old sink is muted, sometimes, but not
always, the level of the new (unmuted) sink is shown. Switching to the
muted source still wrongly shows the old level.
