I have experienced this bug many many times. The laptop I am using is a
Lenovo ThinkPad P71.
I have removed the SSD from my laptop, shut down, connected an external
hard drive, and booted from a USB with Ubuntu 16.04. The installer
obviously recognizes the external hard drive as the main storage device.
I do this because I want Ubuntu with full disk encryption installed on
an external hard drive. When I get to the screen that asks for a
username and password for the computer, I input the information. Then I
proceed and it loads for about 10 minutes before doing anything. Then
the installer proceeds for a while, and in the bottom left it shows what
the installer is working on. When it gets to "Creating user..." it
crashes 99% of the time.
After attempting to install over 10-15 times, it occasionally does NOT
crash, and installs correctly at this step. I have no idea why...
This bug DOES NOT happen if I do not select encrypt the disk.
