Apologies, I went on vacation for a few days. I've had the same results no matter where I plug it in, but it seems to happen less frequently on my powered USB hub, and MUCH more frequently when connected to a plain USB 2.0 port (labeled for keyboard and mouse usage on the back of the mobo). It still happens when connected to the regular USB 3.0 ports, and when connected directly to a 2.0 header breakout cable.
I am planning on getting a PCIE usb expansion card for unrelated reasons but I will give that a try when it gets here. The only other thing that I can think of that might be significant is that when running off the 3.0 hub, it seems to have a small chance of failing whenever a new pulseaudio client appears (like when launching a game for example) and does not seem to fail randomly like it does on the 2.0 ports.
