Per upstream this is expected behaviour, if not a bit verbose or
improperly worded. They will eventually dial back the language to make
it less worrying.

Perhaps the word “failure” is not appropriate in log messages that report
informational SAN activity meant for a sysadmin. We will plan to post a
version update for lpfc that reduces the amount of default SAN activity
information. I will keep you updated when this new version is posted.
