Your details highlight IO errors which are usually hardware specific; so have you explored this? If it's a software config; testing your system with 'live' media is probably what I'd do (live media being run from thumb-drive & not your optical drive) and see if the IO errors are occurring there, esp. using a different software-stack to your installed system as its hardware I'd be testing.
