Your machine has a NVIDIA GPU and you're using the free nouveau driver for it. This driver is known to not be very stable yet, and indeed the journal file shows multiple instances of the driver failing on resume from sleep. I suggest you install the proprietary NVIDIA driver by opening the "Additional Drivers" app and installing the recommended one.
