Looks like the Bluetooth link for calls comes up fine, but no audio packets cross it in either direction. The audio stack waits for the headset to send the first packet (a workaround USB adapters need), it never arrives, so nothing is ever sent back, and after a few seconds the system gives up and switches back to A2DP.
