I confirm this bug.
Problem still exists with latest fast-user-switch-applet version 2.22.0.
After starting users-admin through fast-user-switch-applet the "old"
password-prompt appears. It seems that users-admin is startet with sudo
command. If I manually start users-admin with sudo command, I get the
same "corrupted" users-admin window, that can't be unlocked (conflict
with PolicyKit)!
