It happened to my when clicking to download a google video from http://video.google.com/videoplay?docid=4662636869909322164&sourceid=docidfeed&hl=en
I tried to download in windows format, after a while the save-as dialog started to appear - the borders showed but that was all (this happens from an auto-refresh I think from submitting the download form).
