Go to http://wiki.ubuntu.com/'yourname' (e.g. wiki.ubuntu.com/mistrynitesh). If the name is already taken by someone else, his/her page will come up. If not 'This page does not exist' page comes up. On that page there is a list of templates you can use. There is a 'homepage' template also.
