at $JOB[-1] we had a contractor simply rest the fibres in the SFP hole, because he didn't realise the switch actually needed something plugged in! He didn't tell anyone, and time was wasted trying to work out, remotely, why the link hadn't come up. There was a lot of facepalming that day.
