Then wouldn't it be possible to take the client in on a l2vpn / p2p ethernet connection, and hook this up to a physical cross-connect into the NTT service? It's more expensive, but there's no reason that it wouldn't work.

Maybe part of the issue here seems to be that NTT isn't in the business of providing low speed transport services, e.g. anything less than 10G carrier.
