The story you tell here is not “fairly good”. It’s a disaster that took “many weeks” (your words) to fix and degraded the end users’ service. People get bad performance evaluations, lose their bonuses, are overlooked for promotion, get fired, etc, for stuff like this. Your story is exactly why experienced managers will not touch v6.
