I've used different versions of rawrite/ntrw, and rawread to write the
prebuilt images to a floppy disk, modify and test them, and then make a new
image file.

The best version I've found is rawwritewin
    http://uranus.it.swin.edu.au/~jn/linux/rawwrite.htm

This is a GPLed windows app, that will both write and read images, with either
a command line or a GUI interface.
