To be sure of what's happening you need to get a log from a command line so that the message from mplayer.sh are captured. As the problem is in mplayer.sh - you won't see any difference in the slimserver log.
I expect to see one of the following messages in the log - as these come from mplayer.sh Local File: Launching Streaming: Launching If my changes was added correctly you'll see the "Local file" message.
