Older slimserver versions can be found here:
http://www.slimdevices.com/downloads/

Select the one you want and click through for the .deb version,
download and use GDebi to install. (ver 6.5.0 onwards has a .deb
version)

That should give you the init.d startup file you want.

It would be wise to purge the old versions from your system, and start
from scratch.
