Re 1-3 could you post more info. The -z option should work, but perhaps the command line options are not being parsed with the exact options you are using. The naming option looks like it is picking up the name you have set for a player of that mac address before - this is the default behavious of LMS so this explains where "squeezeslave 2" could have come from.
