There's a trick to using the combo USB cards.

USB and Firewire cards need to be present during the install of OS 9 for the drivers to be installed.

The USB drivers in USB support 1.4.1 are for OS 8.6 and lower. You should be able to do a custom install of just USB and Firewire support from your system CD, or re-apply the 9.22 Combo update.

Doing a custom install from the CD doesn't even require the hardware, since I've done that to get the OS 9 version of iMovie to work on systems without a firewire port. (If the FW support library isn't there, iMovie bails on an error right after starting up)

This is all that was needed for the combo card I got from OWC.
