I use a hybrid of a ticket tracker with time logging. Some custom reports, a  
bookkeeper, goolgle docs, and a separate invoicing tool.

The expense of a bookkeeper far outweighs my reservation of a 1 tool fits  
all. But I could be sold on anything I guess. As far as rates, I don't feel  
dishonest getting over paid, so id say as high asyou can!
