All GTK+ programs change their layout when you switch themes. Can't you listen somehow for that change signal? I really like the instant apply. IMO it should stay this way. Instant apply with something that shows a busy cursor for 1+ secs will need a busy cursor. However, when changing the theme the user does see progress (all the apps change their theme). IMO even if this isn't explicitly said in the HIG, it still is in the spirit of it.
