I don't have a strong opinion on how this should be, but of these choices the header approach is the most appealing. It doesn't change the usability of menus / filechooser items, it cannot be thought to erase the data from the media nor does it clutter the notification area with possibly stationary icons (I have an external USB hard drive that's almost always connected)[1]. It does have it's downsides though, imagine removing a spun-down CD after all disk caches have been already cleared. To eject you'd need to open a folder on the CD and for that the data needs to be read and thus it spins up which can take relatively long with the spinning speeds being so high, unless the motor technology has recently seen drastic improvements that I'm not aware of.
