That question came up just last night at a programmer meetup. Everyone present agreed that it's just the nature of the task.

Like Bob said, it's not a failure of any given tool or language, just a failure of the imagination to conceive of all of the possible issues with the many disparate subsystems any project will encounter on its way to completion.
