Yes, and worse. On one occasion I was feeding in trays and trays of cards that represented free gifts to the clients. Although I was always very methodical and careful when doing this, somehow, I don't know how, I fed in a tray (or more?) of cards twice. As a result, about 10,000 customers got their rather expensive gifts in duplicate! Needless to say, proper statistical controls were implemented by my department afterwards. It turned out to be quite didactic.
