I would recommend grabbing the Live HTTP Headers Extension/Plugin for Firefox (http://livehttpheaders.mozdev.org/) and watching what is returned for the Content-Type header, and the content-disposition header. I would suspect that most likely they are all returning an 'audio/mpeg' value for the Content-Type header but the ones that are prompting you instead of being played directly probably have a content-disposition header with a value of 'attachment;' followed by 'filename=[name of the file]'. It could be that the Content-Type being passed across on some are something along the lines of 'application/octet-stream' which may cause Firefox to prompt you to save the file.
I have very little understanding of the dms in relation to the core of magnolia, but in the config/server/MIMEMapping you can see what mp3 is mapped to (it defaults to audio/mpeg). The sporadic nature that you're reporting makes me wonder if maybe you have apache or some other web server sitting in front of your app server which may be occasionally altering or stripping out http headers. If not, perhaps there is a bug in magnolia in regards to it correctly returning the right headers.
