Does the host have a prefix set? If so, you must include that in your

In this case you probably don't want the code-init element in the style,
so
I think that code-init element can be removed. Yes, you can attach
elements
to a page (Page->[select page]->Elements). These will behave like normal
elements but will apply only to that page, or lower pages if you enable
element inheritence.
