Could have multiple reasons:

- you set auth on for individual pages, but not the host record
- you have set on for a prefixed host record and the 'root page'
  you try to access actually matches on a non-prefixed host record.

There could be other instances. Set LogLevel to debug, and include all
the debug infor for a single request that fails, and for a single
request that succeeds.
