$prefix/lib/libmidgard.*
$prefix/include/midgard/*
`/usr/sbin/apxs -q LIBEXECDIR`/mod_midgard.so
`/usr/sbin/apxs -q LIBEXECDIR`/libphp*
$prefix/bin/repligard

and drop the database.

To be sure, I'd update my locate database and do
# locate midgard | grep $prefix
