Whatever this message says :) it means that PHP didn't
load the extension from php.ini. Highly recommended.
If you translate the above error we'll know why.

Fatal error: Call to undefined function: mgd_get_midgard() in
/usr/local/apache/libexec/midgard-root.php on line 14

That error is a result of PHP failing to load Midgard.

Is this the same problem as referd to under compiling pros
with midgard-php4

?
(I'm using php from cvs, should I use php-4.0.4rc1?)

That I don't know. The errors you quoted are probably solved by
the new php4 package available from the midgard site (including
a generated header file did the trick). I'm trying to update
sourceforge but their FTP server is overwhelmed it seems.
