It looks like it is uploading the image as an attachment to the attachment
page rather than it's parent.. 

if you can get mysql logging going, it might be worth 
tail /var/log/mysql.log -f > /tmp/mysql.log 

while visiting the above URL's - also attempt to upload an image..

- try and use 1 log file for each action so the 'real activity' ends up near
the top of the file.

Its one of those 'not to easy to see by email' problems i think...
